They include but are not limited to: laboratory draw station, pap smears, joint injections, ultrasound, DEXA, retinal eye exams, sleep studies, balance testing, flu and pneumonia vaccines, ABI, DPN, weight management, pre-op clearance, smoking cessation, diabetic teaching and family planning to name just a few.   • Dana Forte DO - BIO
A true Las Vegas native, Dr. Forte was born and raised in the same community where she serves her patients since 2001. She earned her medical degree from the College of Osteopathic Medicine of the Pacific Western University of Health Sciences, Pomona California. Dr. Forte's practice focuses on family medicine, adolescents and student athletes as well as senior care and preventative medicine.

  • Christine George MD
Dr. George received her degree from St Georges University School of Medicine, Grenada West Indies. She also has a BS in Biology and a BA in Religion. Dr. George has been involved in training, lectures and publications on LGBTQ health. Dr. George enjoys Yoga and also has formal training in the techniques. Dr. George has a friendly open disposition and is a favorite Provider in our Henderson office with both staff and patients. 

  • Del Gardner DO
Dr. Gardner proudly served our military as a Military Medical Officer while working at Nellis AFB before joining Forte Family Practice in 2015. He earned his medical degree from the University of Osteopathic Medicine and Health Sciences Des Moines, Iowa, now called Des Moines University. Dr. Gardner is an active member of The Church of Christ of Latter-day Saints.

  • Emilia Boffi DO, PH.D.
Dr. Emilia Boffi is a board certified Internal Medicine Physician. She received her Medical degree from Michigan State University. She has a PhD in Medical Anthropology. Dr. Boffi has a holistic approach. She enjoys spending time with patients and provides them with an individual approach. She likes the continuity of care and is ready to establish her patient base. She is able to speak both Spanish and Portuguese and has a special interest in global health. She enjoys academic research on her own time and publishing. Dr. Boffi has a quick smile, friendly personality and is a great addition to our practice. 

  • Itha Dalrymple DO
Dr. Dalrymple received her Osteopathic degree from Touro University of Nevada. She also holds a Masters of Medical Health Sciences as  well as a degree in Biology. Dr. Dalrymple was also the President of the Student Osteopathic Internal Medicine Association and Vice President of the Asian Pacific American Medical Student Association. Dr. Dalrymple is married to a fellow Osteopathic Physician in our practice and they have a young child. Dr. Dalrymple loves spending time with her patients and is a thorough, friendly provider. She is a great addition to our practice.

  • Michael Yudez DO
Dr Yudez helped to create the largest Urgent Care/Family Practice in the North Denver Metro area before coming to Las Vegas in 2016. He earned his medical degree from the University of Osteopathic Medicine and Bio-science, Kansas City Missouri, where he was ranked 8/165 in his class. Dr. Yudez prides himself on his community involvement and speaks often at local senior centers. Dr Yudez is the most requested provider at our Henderson location.

  • William Dalrymple DO
Dr. Dalrymple received his Osteopathic degree from Touro University of Nevada. He also holds a Bachelor of Arts in Biology with a minor in Chemistry. He was the Medical Education Chair for the Student Osteopathic Internal Medicine Association and the Interdisciplinary Liaison for the Asian Pacific American Medical Student Association. Dr. Dalrymple is married to a fellow Osteopath in our practice and they have a young child. Dr.Dalrymple is easy to talk to with a kind smile and caring personality. He is a great addition to our practice.

  • Candace Chow MD CCFP
Dr. Chow received her degree from University of Manitoba, Winnipeg, MB, Canada. Dr. Chow likes to figure out the patient's needs and doesn't want to tell them what to do. She likes the approach of providing them lots of information so they can make the best choice for themselves. She thinks of herself as a patient advocate. She describes herself as a jack of all trades and can do a little bit of everything in family medicine. Dr. Chow is very friendly and enjoys forming lasting relationships with her patients. 

  • Gregory Dyer PA-C
Greg Dyer received his degree as a Physician Assistant from the University of Nebraska. He proudly served our country in the US Coast Guard as a Hospital Corpsman. Greg has a long history of providing quality healthcare to the residents of Las Vegas. He has extensive experience managing chronic medical illness and is very procedurally oriented. Greg has certification for CDL physicals. Greg has built a loyal patient base and gladly welcomes new patients. In his spare time Greg enjoys spending time with his family and building computers. 

  • Kyle Anderson PA-C
Kyle received his Physician Assistant degree from right here in Las Vegas at Touro University. Kyle is an active member of the Church of Christ of Latter-day Saints. Along with completing his mission service in Milan, Italy, Kyle volunteered with the 4-H program for children. Kyle was an Eagle Scout so you can see service to others is important to him. A proficient practitioner, Kyle has an easy going personality. Kyle's hobbies include woodworking, automotive repair, welding, fishing, hiking, camping, football and spending time with his wife and two children. 

  • Beth Moreno APRN
Beth Moreno joined Forte Family Practice after a dedicated career as a Emergency Room Trauma Nurse. She received her degree from Chamberlain College of Nursing, Chicago, Illinois. Advanced Registered Practical Nurses function in the same capacity as Medical Doctors and Doctors of Osteopathy. They can diagnose and treat patients, order testing and have prescribing privileges. APRN’s have achieved the highest level of training Nurses can possess. Provider extenders are an important part of Forte Family Practice. Beth Moreno has a very caring and  friendly practice style.​

  • Brittney Turner APRN-C
Brittney received her advanced nursing degree from University of Nevada, Las Vegas. Before obtaining her APRN degree Brittney was an ER nurse. Healthcare runs in Brittney's blood, her father is long time Las Vegas OBGYN Mark Turner. Brittney is an active member of The Church of Jesus Christ of Latter-day Saints, serving in Santa Cruz, Bolivia and Boston. Brittney is a smart, motivated practitioner and is accepting new patients.

  • Elizabeth Kelemen APRN-C
Liz received her Nurse Practitioner degree from Graceland University, Independence, MO. Prior to her advanced degree she was an emergency room and trauma nurse. Liz is a huge Vegas Golden Knights fan. Liz is happily married with a teenage daughter. Liz is very friendly and energetic, she also speaks fluent Spanish. 

  • Sharon Del Rosario Aquino
Sharon received her Physician Assistant degree from Touro University in Henderson, NV. She speaks fluent Spanish. Before obtaining her advanced degree, Sharon was an emergency room RN. Sharon co-founded the Latino Medical Student Association. Sharon has a long history of volunteer work with many local organizations. She enjoys spending her time with her young son. Sharon is a very passionate provider and is a terrific addition to the practice. 

  • Sherri Yagoubi APRN-C
Sherri received her advanced nursing degree from Graceland University. Prior to that she was an emergency room RN. Sherri also taught at the College of Southern Nevada as well as Nevada State College. Sherri has an infectious smile and an easy going personality. A proficient practitioner, Sherri has been a welcome addition to our Rainbow location.

  • Joseph Eafrate PAC
Joe has been a staple of Forte Family Practice as well as the Las Vegas community since 1995. Joe was trained in medicine as a United States Navy Hospital Corpsman. Joe is an outstanding provider with a wealth of knowledge. Joe is detailed oriented and his patient care is on point. Joe feels a passion for addiction medicine and has been successful in helping those with opioid addiction.
